Akute health is the electronic medical record (EMR) that Coastline uses to provide patient care.  Akute has a patient portal that allows for secure messaging- you should receive a link to this within 48 hours of registering (let us know if you do not get this).

Akute Health Intake Forms

The below forms will help us gather and populate your chart in Akute Health. By collecting this information ahead of your first visit, we will be able to review the information and gather additional information and schedule a plan of care as appropriate. Additionally, please request that your current health care provider(s) forward us your medical records. You can download a release below.
